I'd prever a combination of cron and a pid file.
Don't like long running scripts with long sleeps, for those kind of scripts the cron was invented. I can add an extra script to the package and let appinit register an crontab line to periodically check if the pid file content is still a nzbget. the start/stop script should create / remove the pid file. Hmm, even better, I can check what the startstare is using appinit and check if a nzbget is still running. If not restart the application.
